A feasibility and components survey for one (1) computer work station at any work location ( remote or normal ).
This survey was designed to help assist an employer and employee to determine the feasability of remote working, inter alia for establishing a proper work station for the purposes of working from home or remote working, by ascertaining if the work components and conditions that are currently in place (or not) are suitable and fit for purpose and if equipment or conditons gaps may exist or need to be overcome. Employers and employees need to separately consider the suitability of the role and function for remote or blended work and test the broadband connectivity and associated download/upload speeds.
Completion of this survey will not of its self satisfy an employers obligations under SHWW law for DSE users or the employers duty of care obligations. This survey is structured to primarily cater for working from home/remote working however it will equally cater for the essential components for establishing a normal office work station conditions. e.g. it may be used for either situation and is dual purpose.
One complete survey per work station may be entered via this link. The survey may be paused at any time and resumed later until fully complete. Progress will be saved. A submitted survey can not be edited subsequently.
This is a structured stage 1 work station component survey only. It is not a full work station assessment nor a full home working assessment.
The input questions/survey is designed to impart knowledge and will assist (you) the user to identify the necessary components which are essential for setting up a computer work station and then conducting a full work station assessment at a later stage.
This work station components survey is formulated in three (3) discrete parts –
Part 1 = Preliminary information
Part 2 = Work station components – your current equipment
Part 3 = Work place – general information
Part 4 = Summary/ end of survey feed back – The key work station component gaps identified.
Note:
- All answers must relate to the work location selected.
- In the case of blended work at over two (2) or more locations ~ a separate component survey will need to be completed for each workstation. i.e. complete separate 4 Front Safety component surveys. [ In such circumstances please contact your employer to order a separate survey via 4 Front ].
